Primary School Applicants: Does your child require diaper changes?

Our nursery school class is for our youngest learners who are not yet potty-trained. Our preschool class is for three to five year old children who can independently use the toilet.

Financial Information

Are you requesting a monthly tuition payment payment plan?

As a convenience for our families, we offer a monthly payment option for our school year programs. Families can choose to pay tuition with 10% down and nine monthly installments. Monthly payments are due on the 1st of each month (September through May). Monthly payment plans require the approval of the business office prior to July 1st and the 10% deposit must be paid by August 1st.

Parent Acceptance of School Policies

Uniform Dress Code *

I have read the school uniform dress code policy and understand the casual and dressy options available. I will ensure that my child is in compliance daily.

Parent Interactions *

I will be courteous and respectful when interacting with staff, students, parents and anyone else in the school community.

Student Behavior *

I understand that Nahunta Hall has student behavior policies in place, including a Safe Schools Policy. My student will comply with all such policies and procedures. I understand that Nahunta Hall has a zero-tolerance policy for bullying and harassment. Disrespectful words or actions are unacceptable, particularly those focused on race, ethnicity, sexual orientation, gender, religion, social class, appearance, or disability, and will result in severe administrative action as allowed by school policy and State law.

Pick-up and Drop-off Procedures *

I will obey all school safety procedures relating to transportation to and from school. I will follow all safety instructions given by school staff. I will drop off and pick up my child on-time.

Photo Release *

I give Nahunta Hall permission to use images of my child taken at school or school approved events for any purposes related to Nahunta Hall. I understand that these images may be used without any further notification. I release Nahunta Hall and its governing board, employees, agents, representatives or assigns from all liability which may arise from any and all claims or demands made by me or any third party ensuing from or in connection with my child’s participation, including any and all claims for libel and invasion of privacy.

Infectious Disease Control *

I have read the Utah Health Department regulations and Nahunta Hall policies regarding student illnesses and I agree to keep my child home when they are ill.

Parent Involvement *

I want to be an active partner in my child’s education. I will contribute to school committees as my time and talents allow. I will fulfill 36 volunteer hours per child per year. I will attend parent teacher conferences and parent education workshops.

Communication *

I understand that Nahunta Hall will utilize email and other electronic technologies as the primary means of communicating with parents and students. I will provide accurate email addresses, phone numbers and text capable cell numbers whenever possible.

Termination of Enrollment *

I understand that Nahunta Hall may terminate the enrollment of a student for failure to comply with attendance, academic, behavioral, or financial requirements. A student choosing to withdraw must provide written notice prior to August 1st. Failure to provide written notice of withdrawal by the deadline will result in a continued obligation to pay the full yearly tuition regardless of attendance. The materials and activities fee is nonrefundable.
